Today we celebrate the birth of a little girl who would be called to become the Mother of her Creator, the Mother of God. Yet, she is born simply as a little girl, like the daughters and granddaughters we hold in our arms. We bestow upon her the most exalted and noble titles, such as Our Lady and Queen, but God’s way was and remains simpler and quieter, yet no less powerful and transformative. It is the way of Jesus Himself. #NativityOfMary
#weekend_review#leusm#venetoclax_4_AML 🧵
1/10 Venetoclax duration in #AML: toward a precision approach
Ven-HMA is standard therapy for older/unfit AML, but 2 questions remain unsettled:
https://t.co/v31e1TZxBh long should venetoclax be given within each cycle?
2.Can treatment eventually be stopped after a durable remission?
Increasingly, the answer may depend on genotype + depth of response, rather than one schedule for all.
True mastery in medicine is not measured by how complex you can make a subject sound, but by how cleanly you can demystify it.
Too often, medical education falls into the trap of the esoteric.
Academic discussions gravitate toward the one-in-a-million zebra, obscure edge cases, and hyper-niche anomalies. While intellectually stimulating, this fixation often leaves frontline clinicians without the practical tools they need most.
Generalists and trainees do not need specialists to demonstrate how rare a disease can be; they need actionable clarity to master the 90% of conditions that walk through the clinic door every single day.
Simplifying a complex specialty is never about "dumbing it down"—it is about distilling nuance so effectively that frontline physicians can recognize and treat the vast majority of cases without second-guessing their clinical instincts.
My father always set a profound standard that shapes every presentation I build: Before you speak, ask yourself whether you are lecturing to show the audience how much you know or truly trying to teach them something. The first choice serves the speaker's ego. The second choice serves the patient. I apply the same approach to review articles I write.
When experts step off the podium of academic prestige and into the role of a supportive collaborator, the standard of care as a whole rises; we do not need more grandstanding around the 10% exception; we need bridges built between specialty depth and generalist practice. When we focus on equipping clinicians to conquer the core 90%, we don't just create better learners—we protect the patients who rely on them.

#Weekend_review#menin_inhibitor#leusm 🧵 Menin inhibitors in AML: biology → approvals → combinations → resistance
1/9
Menin inhibition has rapidly moved from a biological concept to an established therapeutic strategy in AML.
Why does it work?
KMT2A-r and NPM1-mut AML are genetically distinct—but both converge on the HOXA/MEIS1 transcriptional program, which maintains leukemic stemness and blocks differentiation.
Menin helps sustain this program.
Menin interacts with KMT2A and helps maintain transcription of genes including HOXA9 and MEIS1.
Blocking the menin–KMT2A interaction suppresses this leukemic program and promotes myeloid differentiation.
Importantly, mutant NPM1 also creates a transcriptional state dependent on this axis:
KMT2A-r → MENIN/HOX-MEIS ← NPM1-mut
Think of menin inhibition fundamentally as a differentiation strategy, not simply cytotoxic therapy.

🚨 AML in 2026 is no longer “one-size-fits-all.”
We’ve moved from static risk → dynamic, MRD-driven, biology-informed care.
Here’s what’s changed 👇 #AML#leusm
Glad to share our @Yale tour-de-force review of #MDSsm published in @Nature Reviews of Clinical Oncology (Free online read at https://t.co/GTWWl9intQ) which has to be the most comprehensive review we have ever written on the subject covering all relevant areas from epidemiology, pathogenesis, diagnosis, risk stratification, to current and evolving therapies for lower and higher risk disease @YaleCancer@YaleHematology@YaleMed
When it comes to Cancer of Unknown Primary, we are trying to answer “What it is” and “How to treat”? IHC markers (and NGS can help). Two main slides by @OncoThor and @hfuentesbayne that we refer to during our CUP treatment algorithm discussion!

CCND1 immunohistochemistry shows high sensitivity (96%) and NPV (95%) for detecting t(11;14) in AL amyloidosis; even with low plasma cell burden. A practical screening tool when FISH is limited. @BU_Amyloidosis

FDA approvals of drugs for myeloma since 2006.
Who could argue that this is not a story of success?
This shows the benefit of the Accelerated Approval Pathway. The two drugs withdrawn had minimal impact on safety or outcomes. What a privilege to be part of this era.
