Pay attention to available dosages of meds you prescribe frequently.
Ex.: furosemide comes in 20-, 40-. 80-mg tabs. So 60 mg BID is annoying for pts to take- choose 40 BID to 80 BID.
Little details -> less confusion, more adherence = win-win!

DKA not seen in HFrEF patients without diabetes treated with SGLT2i
Among those with T2D in DAPA-HF, 3 out of 1075 treated. No DKA reported in EMPEROR-Reduced.
1% increase in genital fungal infections.
Overall AE same or higher with placebo, then SGLT2i.
Benefits >> risks.

Just saw my patient who is doing well on Carvedilol 50 BID, Sacubitril-Vsartan 49-51 BID, Spiro 25 and Empagliflozin. EF previously 15% now 49%. This made my day!
