👉 A very fast single-tip activation map of TV using the ablation catheter beautifully demonstrates the reentry circuit — in this case, a counterclockwise flutter around the tricuspid valve annulus.

👉 For success confirmation, pacing in the coronary sinus (CS) shows a collision of both activation wavefronts at the CTI line, without any breakthroughs across the block line.

A new RCT puts the controversy to bed?

Caffeinated coffee may reduce rather than trigger atrial fibrillation (AF) recurrence.

Among 200 adults post-cardioversion for AF or atrial flutter:

➡️ 1 cup/day of caffeinated coffee ↓ AF recurrence by 39% vs total abstinence

➡️ No increase in adverse events reported.

💡 Moderate coffee intake could actually protect against AF recurrence in habitual drinkers.

👉 Typical atrial flutter on the ECG:

2.The direction of activation across the atrial septum determines the polarity of the flutter waves in the inferior leads (see blue arrows in the video).

👉 Counterclockwise typical atrial flutter: flutter waves are negative in II, III, and aVF, but positive in V1.

👉 Clockwise typical atrial flutter: flutter waves are positive in the inferior leads but negative in V1.

👉 The therapy of choice and first-line treatment is ablation (cavotricuspid isthmus line). Antiarrhythmic drugs have only moderate and temporary effectiveness.
