Our Kidney Institute of New Mexico research team has a new narrative review in the Clinical Journal of the American Society of Nephrology: “Gadolinium-Based Contrast Agents and the Theater of Safety.”
We argue that the preferential use of macrocyclic over linear gadolinium-based contrast agents rests on flawed logic. The assumption that distinct brands impart distinct risks for nephrogenic systemic fibrosis has never been adequately resolved.
Far too often, observational studies discounted patient safety by ignoring prior exposure to different brands of magnetic resonance imaging contrast agents. These “confounded” exposures are practically routine in contemporary medicine.
“There are published cases of nephrogenic systemic fibrosis developing after the administration of the [American College of Radiology] group II agents.”
There are also reports of non-confounded exposure to macrocyclic agents inducing nephrogenic systemic fibrosis. Symptoms may emerge months after administration. Prophylactic dialysis does not reliably prevent this outcome.
The development of nephrogenic systemic fibrosis after exposure to Vueway further demonstrates that gadolinium dose reduction does not abolish calamitous complications.
Our review argues that simplistic theories linking thermodynamic stability constants to patient safety can no longer withstand scrutiny.

LM- normal appearing glomeruli but proximal tubules filled with eosinophilic granules.
IF- negative.
EM- large number of lysosomes, with degenerative changes.
Lysozyme stain 3+ positive in proximal tubules.
Dx: Lysozyme associated-tubulopathy (myelofibrosis/clinical).
Usually see this in patients with CML.
D/D: light chain proximal tubulopathy, but IF & pronase IF are negative.
60-yr old with chronic kidney disease, pulmonary nodules, and JAK2+ myelofibrosis.
Evaluation showed high levels of serum lysozyme, and lysozymuria.
