76 year old VA patient with acute changes in cognition and difficulty walking. Needed a wheelchair to get to and from the car. Had almost no affect and couldn't answer questions.
Over the last three weeks these are the medications we REMOVED. Today, he looked like a new man.
@dvasishtha And don’t forget to include the patient’s family caregiver. Care coordination is a HUGE problem. The customer isn’t the patient, it’s the patient and his/her family caregiver(s) as @sacjai knows well.
Why #family#caregivers are a critical component of clinical care. They can add a dose of reality to the judgement and opinions of the professionals. #healthcare

As doctors we see a snap shot of patients' lives. We make assumptions & plans that we expect patients to follow.
As a teacher, I saw the reality of families' lives. I realised how unrealistic some of our requests are.

@sacjai That’s exactly right. The negative impact on our employees’ ability to work productively and their health is significant. This strategy worsens their situation, most of whom cannot afford to hire help.
