🫀 Pericarditis imaging: from diagnosis to therapy guidance
Pericarditis is no longer just a clinical diagnosis.
👉 It’s becoming an imaging-driven disease.
🧠 The paradigm shift
We used to ask:
👉 “Is this pericarditis?”
Now we ask:
👉 “What type, what activity, and what stage?”
➡️ And imaging is the answer
📊 Each modality has a role (and none is enough alone)
🫀 Echo = first step
Effusion
Tamponade
Hemodynamics
👉 Fast, available, essential
❌ But blind to inflammation
🧬 CMR = game changer
👉 Best tool for inflammation
Edema (T2)
LGE → neovascularization/injury
Sensitivity up to 85–95%
💡 Key insight:
LGE ≠ just presence
It’s extent + intensity + evolution
➡️ Defines:
Active vs chronic disease
Reversible vs fixed constriction
🧱 CT = structure
👉 Best for:
Pericardial thickness
Calcification
Surgical planning
➡️ Especially in chronic / constrictive disease
🔥 PET = inflammation (when unclear)
👉 Detects metabolic activity
Useful in:
Refractory cases
Systemic disease
Diagnostic uncertainty
➡️ Not first-line, but high-value in complex patients
🧩 The key concept: imaging biomarkers
6 pillars (see Figure on page 10 👇):
Edema
LGE (neovascularization)
Thickening
Effusion
Constriction
Fibrosis/calcification
➡️ This is how we phenotype pericarditis
💊 And this changes treatment
👉 Imaging is now guiding therapy:
High LGE / edema →
➡️ prolonged anti-inflammatory therapy
➡️ consider IL-1 inhibitors
Persistent LGE →
➡️ escalation
No LGE + fibrosis →
➡️ think surgery
⚠️ Important nuance
👉 Imaging ≠ everything
Normal imaging ≠ no disease
LGE ≠ always active inflammation
Must integrate:
Clinical picture
CRP
Symptoms
🧠 Clinical take-home
We are moving from:
❌ “treat symptoms”
➡️ to
✅ “treat imaging phenotype”
🔥 Big message
Pericarditis is now:
👉 an imaging-guided disease
And CMR sits at the center of this shift.

A great resident or fellow learns to navigate uncertainty, communicate clearly, prioritize well, and balance self-care with professional duty. They take ownership, remain teachable, and design care around the individual patient. Just as importantly, they understand exactly which studies support current practice, including who was studied, what treatment was given, and what outcomes were measured. Knowledge matters, but so do reliability, judgment, humility, and the ability to connect evidence to the person in front of you.
With deep sorrow, I received the news of Dr. Eugene Braunwald’s passing, one of the greatest figures in world cardiology.
His scientific legacy, clinical vision, and immeasurable contribution to generations of cardiologists will continue to live on in our daily practice and in the history of medicine.
My heartfelt condolences to his family, friends, students, and to the entire cardiology community. May he rest in peace.
Nothing hurts more than being disappointed by the one person who you thought would never hurt you. I thought am important not knowing that am fooling myself.
The nature is smart
Blood doesn’t crash into the LV, it glides.
The anterior mitral leaflet quietly shapes inflow into a smooth arc, guiding it straight toward the LVOT.
Less turbulence. Less wasted energy. Elegant design.🍷🍷
