🔹 Key insights:
• Real-world data: older/frail patients derive less benefit from intensification
• ARPI improves OS & PFS overall → but effect varies by age and biology
• Age matters:
→ <75 yrs → clear OS benefit
→ >75 yrs → no consistent OS benefit, despite PCSS gains
• Disease context matters:
→ Low-volume disease → less benefit from ARPI, especially in older patients
→ Competing risks (CV, frailty) may outweigh gains
• Toxicity is not trivial:
→ ↑ cardiovascular risk across ARPIs
→ Drug–drug interactions → critical in polypharmacy
🔹 Clinical framework:
• Fit patients → consider doublet/triplet based on disease volume
• Vulnerable → optimize comorbidities first
• Frail → question the value of intensification
🔹 Take-home:
In older patients, the question is not just “does it work?”
but “is it worth it for this patient?”

• Patients don’t “see” PSA → but they see and understand imaging
• Delaying progression is meaningful only if it preserves quality of life
• rPFS is valuable → but must translate into real patient benefit
• OS matters → but not at any cost
• Patients care about:
→ how well they live
→ how long they can delay treatment escalation safely
