The field of inflammation is more confusing than ever, and after CLEAR SYNERGY at #TCT2024, the confusion has reached a new level. The trial found that colchicine did not reduce major adverse cardiovascular events post-myocardial infarction, conflicting with earlier studies and meta-analyses that reported benefits. As if that weren't enough, colchicine was accompanied by an unpleasant side effect. This is somewhat embarrassing, given that the drug is FDA-approved and has a Class IIa recommendation in European guidelines for chronic coronary syndromes (but the new trial was essentially in STEMI).
It's easy to say "anti-inflammatory therapy", but several questions seek answers. First, is there a class effect? Absolutely not: methotrexate and glucocorticoids have not worked, colchicine has shown mixed results, and canakinumab has been effective in CANTOS. Moreover, is it a drug for acute or chronic coronary syndromes? One would expect a greater effect in acute settings, but inflammation also plays a protective role in that context, and this new trial is clearly a step backward. Colchicine is a somewhat rudimentary drug: it interferes with inflammasome formation, but this is a very preliminary step in the inflammation pathway. Canakinumab acts on IL-1, but only patients who can also suppress IL-6 formation show a prognostic benefit.
Ziltivekimab is a monoclonal antibody that antagonizes IL-6. We are conducting the ARTEMIS study to understand more, and the results of CLEAR SYNERGY make the outcomes of this study even more important for clarifying the role of inflammation in acute coronary syndromes and whether antagonizing it brings prognostic benefits.
