Does anyone check zinc levels in cirrhotics with recurrent hepatic encephalopathy? For those who are unaware, zinc deficiency decreases the metabolism of ammonia to urea. @NickPowellLab 49yoF with ‘refractory Crohn’s disease’. After ‘failing’ steroids, AZA + IFX (10 mg/kg Q4W), VDZ (300 mg Q4W), UST, more steroids, she was screened for a clinical trial (filgotinib) and finally a diagnosis was made… @MondayNightIBD #DistanceMedEd #GIBoardReview #FOAMgi

48 year old male with multiple "fibrotic" areas in the duodenum on endoscopy and also noted at prior lap cholecystectomy for gallstones. He had NO GI symptoms. This is a duodenal biopsy H&Es and Masson's trichrome.
