#POPULARTAVI -all of these studies will have major impact on patient care & hopefully improve outcomes for CVD
More oddities with #POPularTAVI: the bleeding definitions. They seemed to have used VARC, BARC, TIMI and GUSTO. It makes it complicated to understand actually what they defined as bleeding. Let’s start with the manuscript
@DrMsk1988 Hi! Your thread is ready to read.
#ESCcongress2020 just ended, and I am looking at some of the highlights. The #POPularTAVI trial was https://t.co/8na07EPRpO Have a good day!
#ESCcongress2020 just ended, and I am looking at some of the highlights. The #POPularTAVI trial was a surprise. After looking through the manuscript, a few questions come up.

1.Asa 75-100 mg alone⬇️ bleeding 5.1🆚10%(50% less major bleeding) & no⬆️ thrombosis/stroke🆚 3 mo DAPT
2. No #Yessct for valve thrombosis:3 pts Asa alone & 1 pt dual -both groups 5% ⬆️ gradients
3)older avg 80 yo
4)evolut valve most, no sub analysis
@Argenscore @mirvatalasnag @gonzaeperez @pomyers @PPibarot @GBisleri @GilbertTangMD @rafavidalperez @VictorDayan1 @DrMarthaGulati Interesting issues from #POPULARTavi for me:
➡️Event rate of close to 25% occuring mainly within first 30d
➡️Population at high risk of AF (OAC started in 10-13%)
➡️ still cant rule out subclinical thrombus on valve- no imaging

🩸#TAVR pts older, ⬆️ risk of bleeding
🩸 90% adherence to Rx with ASA alone
🩸 ASA alone better
🩸 Less Life-threatening bleed compared to DualAntiplatelet
🩸 No difference in risk of CVA or MI
Aspirin alone after #TAVI significantly reduced bleeding compared with aspirin + clopidogrel, without increasing thromboembolic events, say latest results from #POPularTavi at #ESCCongress. See what #CardioTwitter influencers say. https://t.co/qDWMXz9L8T
Go With Aspirin Alone After TAVR When Anticoagulation Isn’t Needed

#POPULARTAVI did not surprise!
ASA alone may suffice post-TAVR!
ASA alone as compared to ASA+3m of Clopidogrel reduces bleeding at 1 year in TAVR patients! No difference in rates of thromboembolic events between groups! #ACCEarlyCareer #ACCIC #ACCFIT

#POPULARTAVI #ESCCongress2020: #AAS superior vs. #DAPT c/clopi x 3m tras #TAVI en ptes s/indicación de ACO:
-⬇️Sangrado: 0.57 (0.42–0.77)
-⬇️MuerteCV, Sangrado (no proc), ACV o IAM: 0.74 (0.57–0.95)
-↔️MuerteCV, ACV o IAM: 0.98 (0.62–1.55)

Además de que algo parezca lógico, debe estar apoyado en evidencia. Enhorabuena a los autores de #POPULARTAVI.

💡AAS vs AAS+clopi 3m tras TAVI
➡️Adherencia a clopi 3m=89.2%
➡️13% y 9% inició ACO por FA
🔑AAS solo
🎯⤵️sangrado total, sangrados no relacionados con TAVI, mortalidad
🎯no aumenta eventos isquemicos @escardio @secardiologia @shci_sec @NEJM
#POPULARTAVI
DAPT is a no go after TAVI.
ASA alone better, confirms prior findings: less bleed, no excess ischemia. TAVI pts are at higher risk of bleed due to older age and aortic stenosis itself, procedure alone do not justify intensify tx.
Will Individualization be useful?
#POPULARTAVI: DAPT may not be necessary post TAVR. ASA alone versus ASA+3mos of clopidogrel associated with significantly less bleeding at 1 year with no difference in thromboembolic events between the two groups #ESCCongress @frahman480 @DrAnthonyP @RWatsonMD @RaniKHasan
#POPULARTAVI: Aspirin Alone After TAVI Reduces Bleeding Events Significantly Without an Increase in Ischemic Events #ESCCongress2020 #ESCCongress Really important Study We MUST figure out the Optimal DAPT or OAC for pts undergoing #TAVR @ACCinTouch @HeartValveOrg @escardio

Conclusions #POPULARTAVI #ESCCongress
🎯 ASA alone vs ASA + clopidogrel has ⤵️ bleeding: less major, life threatening bleed or disabling bleed
🎯ASA alone arm has no greater risk of stroke or MI
🔺~ 90% adherence to treatment , 49% women (YAY!), ASA 75 mg
