In some heart failure patients, LV lateral wall delay isn’t just a simple LBBB — it's driven by complex conduction system disease, myocardial scar, or electrical uncoupling.

🧠 Studies show CSP alone can’t always correct this. Even BiV-CRT has its limits due to non-physiological pacing, RV lead effects, and venous anatomy.

💡 That’s where HOT (His-Optimized CRT) and LOT (LV-Optimized CRT) come in.

👉 Consider HOT/LOT-CRT:
✅ At initial implant if CSP or BiVP give suboptimal ECG results
✅ As an upgrade if clinical or ECG response to CRT is suboptimal — especially in non-specific IVCD or mixed conduction disease

🔥 During #EHRA2025, we witnessed some amazing late-breaking clinical trials!

The SINGLE SHOT CHAMPION trial, led by @TobiasReichlin & @CSticherling, was a randomized, controlled, non inferiority trial conducted at two centers in 🇨🇭

💥 210 patients with paroxysmal AF were randomized to either PFA ⚡ or cryoballoon ❄️  ablation, and all received an implantable loop recorder for continuous monitoring and were followed for 1 year. 📡
