One filter that we should run "groundbreaking" studies is whether results will impact cancer control globally.
Sadly, I found little correlation between importance of talks at #ASCO2025 to global implementation. We need to get better, given that 70% cancer deaths will be in LMICs

Let's be honest. There are major issues with surgical training in India - mental harassment and sexual harassment exist. It is a bigger problem than we think it is. We need to talk about this.
On the other hand there are few excellent surgical training programs where surgeons operate quite a bit.

Now there are programs where both types of faculty exist. I have seen the spectrum. I am currently training in a program (MCh Pediatric Surgery) where every trainee performs at least 100 complex tracheo-esophageal fistulas before graduating.

We are hearing horror stories and they need to be heard. Dismissing them is not right in any way.

We also need to acknowledge that certain surgical programs including some leading central institutions and corporate hospitals produce surgeons who cant operate to save their own lives.

This is a systemic problem where surgical training is not audited. We also need to acknowledge the fact that surgical training can be gruelling in every part of the world. However it can be done better without harassment. There are certain fantastic MS/DNB/MCh programs where training is excellent and mentors are brilliant and understanding.

Many of the surgeons here who are protesting against the term "oldies" being generalised are offended that their mentors are being disparaged. We need reforms but the good programs tell us that it is possible.
