As many of our best physios on here will tell you... Quad ACL grafts are not the best and are not ready to replace BPTP as the gold standard.
The QT group had a higher overall reoperation rate (19.2%) than the HT (8.9%) and BPTB (6.9%) groups (P < .001), driven by extension deficit or pain (11.9% vs 5.5% and 4.7%, respectively; P < .001). QT grafts remained independently associated with reoperations for extension deficit or pain after adjusting for confounders (odds ratio [OR], 1.96 [95% confidence interval (CI), 1.44-2.67]; P < .001). The QT group demonstrated lower odds of achieving the minimal important change than the HT (OR, 0.56 [95% CI, 0.41-0.77]; P < .001) and BPTB (OR, 0.48 [95% CI, 0.32-0.72]; P < .001) groups.
Let’s talk about the biggest mistakes people make in the gym.
The first one is confusing complexity with progress.
They show up to a class, or a trainer puts them through a workout with a lot of complex exercises balancing on those balls, and they get sweaty and sore. The next time, they show up and do totally different fancy exercises and get sweaty and sore again.
A year later, they realize they haven’t grown muscle and they’re lifting the same weights.
What happened?
All that complexity made them feel like they did something big.
But doing their fancy exercises meant they were lifting less weight than the person doing the basics: row, squat, chin-up, deadlift, press, lunge.
And changing it up every week made them feel like they were doing more than the person who just stuck to the foundational movements, but it also meant they were sacrificing the key to muscle and strength growth: progression. Adding 5 pounds to the same lift a week is going to make you stronger. When you do something new every day, you miss that.

It’s why when people join the Pump Club app, they do a Foundation program customized for their level and equipment. Beginners and intermediate members love it. Some people who think they’re advanced feel attacked - at first.
“I need a specialized program just for my arms.” (I have never seen someone who can row heavy with small arms).
“I need to train every day.” (Most people need to learn to train hard enough that they need rest before they’re ready for every day, which we do have after the Foundation.)
“I’m too advanced for this.” (I just watched a guy at an app meetup who did the Foundation deadlift 770 pounds in front of me, I guarantee no one complaining can do that).
People want to skip the basics because they’re hard and seem simple. Simplicity is the fastest path to progress.
The popular quote "Don't speak negatively about yourself, even as a joke. Your body doesn't know the difference. Words are energy and they cast spells, that's why it's called spelling" Bruce Lee :
مايك تايسون، في عز مستواه، كان يسوي إلى ٥٠٠ تكرار من هذي الحركة، المعروفة باسم "تمارين تايسون".
هي من أقوى تمارين وزن الجسم، لأنها تشغل الجسم كامل.
وتقدر تمرّن الرجلين والجزء العلوي من الجسم في نفس التمرين.
وإليك روتينه في التمرين ..
The latest Clinical Practical Guidelines for Achilles Tendinopathy recommend exercise as a first line treatment supported by education and an individualised approach. See our graphic for a summary of the recommendations from Chimenti et al. (2024), published in JOSPT:
Choose your words with care!
Patients tend to interpret phrases like ‘degenerative’ and ‘wear and tear’ very negatively.
Check their understanding too - one study found that some patients thought a knee ‘effusion’ meant the joint was fused together! 🤯
@hjluks I share the results of this study, with manu of my physical therapy patients. maybe 60-70% are relieved. many of the others are angry and confused as they want more concrete straight forward black/white diagnosis, answers, healing timelines.
Target the glutes and get the buns burning with these 3 exercises!! 🔥

Many different loading programmes can be effective for tendinopathy. The most effective ones tend to be progressive and tailored to the individual with rehab adapted based on symptom response 🏋️
Does inflammation play a role in tendinopathy? 🤔
We had moved away from it but recent research is suggesting both local and systemic inflammation may be part of the pathology.
What do you think?

Rehabilitation Rules of Thumb ⛑️
1. You don’t get fast🏎️ by moving slowly
2. Explosive💥strength is often protective
3. Explosive strength needs to be trained and properly programmed or it won’t get better!
4. Ax the needs of your patients sport⛹️♂️or 👷occupation
