• Dilated small bowel loops with multiple air-fluid levels (proximal obstruction).
• Transition point at the right inguinal canal — bowel loops herniating into the inguinal region.
• Decompressed distal small bowel and colon.

Teaching Pearls
• Common cause of SBO in adults: Adhesions > hernias > tumors.
• Inguinal hernia as a cause is often missed if not carefully traced to the groin on imaging.
• Red flags for incarceration/strangulation: Severe pain, irreducible swelling, bowel wall thickening, reduced enhancement, mesenteric edema, or free fluid.
Management: Urgent surgical reduction/repair + nasogastric decompression. Delay can lead to bowel ischemia or perforation.

Diagnosis: Massive Right Pleural Effusion (likely malignant given heavy smoking history) 🔥
Key Findings on CXR:
- Complete white-out of the right hemithorax
- Contralateral mediastinal shift (trachea and heart pushed to the left)
- Possible rib destruction (suggestive of underlying malignancy)
Top Differentials (as discussed):
1. Massive pleural effusion (malignant, parapneumonic)
2. Complete lung collapse (e.g., endobronchial obstruction by tumor)
3. Large mass occupying the hemithorax
Immediate Management:
- Urgent chest tube insertion (ICD) to relieve mediastinal shift and improve oxygenation
- Supplemental oxygen / non-invasive ventilation
- Send pleural fluid for cytology, culture, biochemistry, and cell count
- Urgent CT chest (after drainage) to look for underlying malignancy
Teaching Pearl:
In a heavy smoker with unilateral white-out + contralateral shift → think malignant effusion until proven otherwise.
Always confirm with bedside ultrasound before inserting a chest tube if time permits.
