🟢 Clinical Pearls: Diet & Metabolic Risks in India (ICMR–INDIAB Survey-21)
Dietary Profile of India
High intake of low-quality carbohydrates → polished rice, milled grains, added sugars.
Saturated fat consumption is high.
Protein intake is low, especially plant & dairy protein.
Carbohydrate Burden & Risk
Highest carb consumers vs. lowest:
↑ Risk of T2D (OR 1.30)
↑ Prediabetes (OR 1.20)
↑ Generalized obesity (OR 1.22)
↑ Abdominal obesity (OR 1.15).
Whole Grains Not Enough
Merely replacing refined rice with whole wheat/millet without reducing total carb load → ❌ No significant reduction in T2D or obesity risk.
Protein Substitution Matters
Isocaloric substitution of carbs with protein reduces risk:
Dairy protein: ↓ T2D risk (OR 0.89), ↓ Prediabetes (OR 0.82).
Egg protein: ↓ T2D risk (OR 0.91).
Fish protein: ↓ Prediabetes risk (OR 0.94).
Plant proteins show similar benefit.
Public Health Message
Strategies must aim at reducing total carbohydrates and saturated fat.
Increase plant-based and dairy protein intake → can mitigate India’s metabolic disease burden.
