Sharing a case of a left free-wall accessory pathway involving the vein of Marshall, successfully treated with ethanol infusion.

🧠 Learning point:
When endocardial lesions are ineffective and activation appears broad, Vein of Marshall anatomy may be contributing.
VOM mapping and ethanol infusion can be considered in select cases.
