JUST IN: + pivotal trial for PD-1 in non-muscle invasive bladder cancer (NMIBC): The CREST trial of BCG +/- PD-1 sasanlimab in BCG-naïve, high-risk NMIBC met 1EP of EFS. Adding sasanlimab to BCG induction + maintenance shows:
•32% ⬇️of EFS event (HR 0.68, p=0.0095)
•>50% reduction in HG recurrence

Patients with high-risk non–muscle invasive bladder cancer (NMIBC) are typically treated with transurethral resection followed by intravesical BCG, but frequent recurrence, progression, and global BCG shortages underscore the need for new therapies. Promising treatment strategies in the BCG-naïve setting include combining intravesical agents with systemic PD-1/PD-L1 immune checkpoint inhibitors (ICIs), currently being evaluated in Phase 3 trials. As ICIs enter this traditionally urologist-managed space, integrating medical oncology expertise becomes essential, marking a potential paradigm shift toward multidisciplinary care.

Based from international clinical experience, the authors advocate for collaborative workflows involving urologists, oncologists, and nurses, emphasizing effective communication, increased resources, and comprehensive education to optimize patient outcomes and successfully integrate emerging therapies into routine NMIBC care. #BladderCancer
