🧠 Stroke with no clear cause? Look at the left atrium.
New research in @EHJ_Imaging shows that atrial cardiomyopathy — driven by CV risk factors, cardiac fibrosis & atrial remodelling — significantly raises the risk of:
▪️ Recurrent embolic stroke
▪️ New-onset atrial fibrillation
Key finding: LA Circularity outperforms both LA Strain and clinical variables alone as an independent predictor (p<0.001).
➡️ In ESUS patients, echocardiographic assessment of left atrial shape & function could be a game-changer for risk stratification.

Out of 72 definitions, the JACC CV expert panel definition of atrial functional MR was strongly associated with outcomes. @JACCJournals
What are the differences in the Atrial Functional MR definition (ESC vs JACC)?
🫀 1. Left Atrial Size Threshold (LAVI)
-> ESC Definition (More Inclusive / Earlier Stage, Threshold: LAVI > 34 mL/m²
->JACC Definition (More Restrictive / Clinically Advanced)
Threshold: LAVI ≥ 40 mL/m²
🍃 2. Leaflet Motion Criteria (Carpentier Classification)
-> ESC: Only Type I (Normal Motion)
Mechanism: Pure annular dilation
-> JACC: Type I
Mild Type IIIb (posterior leaflet restriction)
Mechanism: Annular dilation + tethering
("Extreme LA enlargement draws the posterior annulus toward the epicardial surface, increasing the annulo-papillary distance".)

CLOSURE-AF trial: In patients with atrial fibrillation, catheter-based left atrial appendage closure has been suggested as an alternative to medication for stroke prevention. Research findings comparing these approaches are summarized in a new Quick Take video. https://t.co/qA9bozShUR

Pulsed field vs radiofrequency ablation for paroxysmal atrial fibrillation: the BEAT PAROX-AF trial
This randomized trial performed in patients with paroxysmal AF did not demonstrate superior efficacy of single-shot PFA vs point-by-point RFA using the CLOSE protocol regarding single-procedure freedom from atrial arrhythmias, despite fewer safety events being observed in this study with PFA.

🫀 Watch out for an innovative meta-analysis of RCTs | LAAC vs OAC in AF:
✅ Stroke/SE/CV Death: HR 0.95 — statistically equivalent (Bayesian 0.96)
🩸 Non-Procedural Major Bleeding: HR 0.58 — significantly LOWER
ADAPT AF-DES trial: In patients with atrial fibrillation and drug-eluting stents, the optimal antithrombotic therapy after 1 year is unclear. Research findings on anticoagulant monotherapy versus combination antithrombotic therapy in these patients are summarized in a new Quick Take video. https://t.co/5x5OrHgGvi

There is an interesting rationale behind studies testing strategies that exclude oral anticoagulation during the first month in patients with atrial fibrillation undergoing PCI, such as MATRIX-2 (evaluating single antiplatelet therapy) and WOEST-3 (evaluating DAPT). 1/4

Withdrawal of heart failure therapy after atrial fibrillation rhythm control with ejection fraction normalization: the WITHDRAW-AF trial
Withdrawal of HF therapy following AF rhythm control for prior AFCM and recovered LVEF was not associated with a decline in LVEF for most patients in the following 6 months.

We authored the editorial “Science Behind the Study” for @NEJM, focusing on the AZALEA-TIMI 71 trial, a phase 2b study of the Factor XI inhibitor abelacimab in patients with atrial fibrillation. Collaborating with the journal’s illustrator on the figure, which explains the mechanism of uncoupling hemostasis from thrombosis with Factor XI inhibitors, was one of the most interesting and enjoyable experiences I can recall in this type of academic work.

Among patients with atrial fibrillation at high risk for stroke and bleeding, left atrial appendage closure was not noninferior to medical therapy in reducing the risk of stroke, embolism, major bleeding, or death at 3 years. Full CLOSURE-AF trial results: https://t.co/qAMC2o36Mi
