A new potential step forward in the adjuvant treatment of clear cell renal cell carcinoma.
At the #EAU26 Game Changer Session, results from the phase 3 LITESPARK-022 trial showed that pembrolizumab combined with belzutifan (HIF-2α inhibitor) significantly improved disease-free survival compared with pembrolizumab alone in patients with ccRCC at increased risk of recurrence after nephrectomy.
With a safety profile consistent with the known effects of each drug and exploratory analyses supporting the benefit across patient subgroups, the study represents the first positive phase III trial combining anti-PD-1 therapy with HIF-2α inhibition in RCC, opening new perspectives for the adjuvant setting.

🔍 Early disease
Active surveillance is the default for low-risk tumors, avoiding overtreatment while maintaining excellent outcomes. MRI-first pathways improve detection of clinically significant cancer.
⚖️ Localized to high-risk
Surgery and modern radiotherapy offer equivalent cancer control. Hypofractionation and SBRT shorten treatment without compromising efficacy. High-risk disease needs multimodality care.
🚀 Metastatic hormone-sensitive
ADT alone is no longer enough. Doublet or triplet therapy with ARPIs ± docetaxel is now standard, with prostate RT benefiting low-volume disease.
🎯 mCRPC era
Treatment is biology-driven: PARP inhibitors for HRR alterations, Lu-177 PSMA radioligand therapy after ARPI and taxanes, cabazitaxel preferred over ARPI sequencing.
🧠 Big takeaway
Sequencing, patient selection, toxicity, and shared decision-making matter as much as drug choice.

When Charrière made instruments, he picked his own unit of measurement that corresponded to 1/3 of a millimeter...this was the 'Charrière'.
But English speakers struggled to pronounce 'Charrière', so the companies changed it to 'French'. So 1 'French' became 1/3 millimeter.
This difference in the way JP's and Blake drains are measured means that a 'size 10' JP drain is actually considerably LARGER than a 'size 19' Blake drain.
Note that the 19 Blake has a 6.33 mm diameter, whereas the '10' JP drain is 10 mm wide. 🧐📏
Finally, as a trauma surgeon, I have to point out one of my favorite teaching points: The flow rate of a 16 G IV is **twice** that of an 18 G IV.
So the difference between those two IV's may not sound much at first, but it makes a big difference for major resuscitations.
