In the STITCH trial, small-bites fascial closure for elective midline laparotomy was associated w/ reduced long-term incidence and size of incisional hernias vs large-bites closure. At 13 years, cumulative incidence and width of hernias >20 mm were lower w/ small-bites technique.
Surgical therapy for #gastroparesis in adults with #diabetes was associated with improved glycemic control, reduced insulin utilization, and fewer diabetes-related complications vs nonoperative management.

Lying in a legal briefing would get you disbarred as a regular lawyer. But when you’re an unelected, lifetime appointee with powers to write decisions that completely undo & remake the Constitution, the decision gets to stand & you get to keep your lifetime position. Outrageous.
As a trauma surgeon I have seen so many horror stories from elderly patients using anticoagulants such as Eloquis, Xarelto and others - I see brain bleeds, hemorrhagic stroke, retroperitoneal bleeding…. But I know that although I may see 10 of these patients every time I am on call…. There are millions of others where these medications have saved lives, prevented stroke, prevented embolization and infarcts and many other beneficial effects. As a physician any patients with a bariatric complication will contact you because you are their doctor- realize you are the front line and you see the complications- we must rely on data and studies that give a big picture that is more accurate than opinion.
Bariatric surgery and GLP-1ra medications used together have an additive effect. This is “the secret sauce” of obesity treatment and long-term control. To think that one treatment should be considered only for a patient sacrifices the chance to achieve the greatest success.
Someone can certainly *make* a billion dollars. That’s not the same thing as earning.
Growing fast and disrupting markets also often means chasing and wielding market power, political influence, and scale.
Take Airbnb. They heavily lobby politicians against passing housing laws to protect working class residents because it’s bad for their business model.
Airbnb could not exist at its current scale and size without the housing market destabilizations, displacements, and exploits that are supercharging the evictions of working people everywhere from Puerto Rico to Jackson Hole.
Now young people are planning for a future where they will never be able to afford to own a home while others have 20 and live off renting it out to them at extortionate rates with zero protections. Yes, a tiny amount of people can make billions of dollars doing that. And millions of everyday Americans are bearing the cost.
