⭐️🔥✅Key points that should be kept in mind in terms of using TMB in clinical practice:🚨
🧬TMB is estimated using next-generation sequencing (NGS), whole genome (WGS), or whole exome sequencing (WES). Targeted panels of ≥0.8 Mb are used clinically, but accuracy drops below 0.5 Mb.
🧬High TMB correlates with better ICI outcomes in cancers like NSCLC and melanoma, but some low TMB cancers like RCC also respond well, indicating TMB alone doesn’t predict ICI efficacy.
🧬Trials show mixed results on TMB’s predictive value for ICI benefit. The KEYNOTE-158 study found a 30% response rate in high TMB tumors, leading to pembrolizumab approval for such cases.
🧬Despite approval, TMB’s role as a universal biomarker is not fully established, with questions remaining about its predictive power across different cancer types.
