A strong MDT gives #HFSNs the support, expertise & collaboration needed to manage increasingly complex patients. It means care is joined-up, safer & more focused on patient needs.
It’s encouraging to see robust MDTs in place, but making this the norm everywhere is vital.
Consistent access to cardiology, renal, diabetes, medicine for the elderly, pharmacists, palliative care & other professionals isn’t optional it’s the foundation of high-quality, equitable heart failure care
