Sharing a practical @OpenMedicineHQ algorithm by @ManniMD1 on Monoclonal gammopathy–associated peripheral neuropathy (MGNS)
General guidance supports systematic evaluation of peripheral neuropathy and recognition that an M-protein may be incidental or related (MGUS/MGNS), prompting targeted testing for: 1) AL amyloidosis, 2) POEMS, and 3) IgM-associated entities.
KEY POINTS (Diagnosis):
- BEFORE ATTRIBUTING NEUROPATHY TO MGNS: Characterize neuropathy (EMG/NCS; consider skin biopsy for small-fiber when appropriate) and evaluate alternate causes (B12 with MMA if borderline, TSH, HbA1c/fasting glucose, folate, CMP, CBC, SPEP/IFE, serum free light chains, quantitative immunoglobulins).
- Define clone and end-organ involvement: CBC, CMP (Cr/eGFR, LFTs), calcium, beta-2 microglobulin (as applicable), LDH, serum viscosity if hyperviscosity symptoms (IgM/WM), and baseline immunoglobulin levels.
- Amyloidosis => Isolated nerve involvement without heart/kidney extremely unlikely.
- POEMS =>VEGF helpful.
