Changes in HER2low and HER2-ultralow status in 47 advanced breast carcinoma core biopsies, matching surgical specimens, and their distant metastases assessed by conventional light microscopy, digital pathology, and artificial intelligence

This study evaluated HER2-low #BreastCancer assessment across core biopsies, surgical specimens, and metastases using conventional microscopy, digital pathology, and an AI model. The AI approach proved more sensitive in detecting HER2-low and ultralow expression, identifying more expressor tumor cells than traditional methods. HER2 expression was found to vary by specimen type, with metastases showing increased expression.

These findings highlight the heterogeneity of HER2-low tumors and underscore the need for standardized, high-sensitivity assessment methods to better guide eligibility for emerging HER2-targeted therapies such as trastuzumab deruxtecan.
