A patient with MHx of COPD is admitted from the ED hypotensive, on pressors (norepi 0.1, after receiving 2L NS), likely septic from "UTI" (UA looked "dirty"). You just began teaching the med students the use of IVC diameter/collapse for "fluid management". You get this IVC view:

Adhikari's Image of the Wk: Subhyaloid Hemorrhage.
A collection of blood in a previously nonexistent space, usually between the posterior limiting layer of the vitreous and the retina. Sonographically, hemorrhage appears ‘D’ shaped #POCUS
