From Silos to Systems: Rethinking Circulatory Health
Circulatory health is the next big frontier in public health, and its time for a new model
We rarely see kidney disease, heart disease, diabetes, or hypertension in isolation. They overlap, yet health systems still treat them separately. That siloed approach is failing patients.
For the past seven years in the #UAE, I’ve been testing and refining new ways of caring for these patients using healthcare informatics, digital tools, bedside ultrasound (POCUS), and now #AI. It’s been a journey of refining, failing, and improving, but the thesis is clear:
👉 Circulatory health needs a new model of care.
Not one that waits until disease shows up, but one that can see across domains, predict risks, and intervene earlier.

Redesign Care Delivery, Digital First, and Tech-enabled
The future of nephrology isn't just about better treatments; it's about fundamentally reimagining how we deliver care.
Seven years ago, I had a career-defining moment while spearheading ESRD digitalization at SEHA - Abu Dhabi Health Services Co. The traditional renal care model we've inherited just isn't working. Not for patients, not for healthcare systems, not for anyone.
That realization sent me down an unexpected path.
Last week, I presented at the Indian Society of Nephrology - Southern Chapter Annual Congress in Vijayawada, using the landmark study by Chinta Rama Krishna, Bogdan Milojkovic, MD, et al. on point-of-care testing and digital health technology for renal screening.

What I've Learned Building Digital SOPs Over 7 Years
Most healthcare "innovations" fail because we digitize broken processes. Instead, we rebuilt from scratch:
a. Reliable protocols eliminating clinical variability
b. Scalable systems that don't collapse under volume
c. Efficient workflows maximizing resources without compromising care
The Validation Moment
Being recognized not just as "another good nephrologist" but as a systems thinker and healthcare innovator was life-affirming—validation of 7 years pioneering work in #UAE healthcare.

The Reality Check
I trained under elite nephrologists worldwide—that remains my calling card. But learning to speak both medicine and technology has opened doors to real-world impact.
To fellow clinicians: The coming years will separate adapters from non-adapters. Clinical expertise gets you to the table, but tech navigation determines your influence.
The Uncomfortable Truth
If clinicians don't lead healthcare transformation, someone without clinical insight will. Technological gains only translate to patient impact when clinicians drive change.
This shift changed my career narrative. While clinical training provided the foundation, my tech adoption reputation opened doors to system-wide influence and collaboration on real-world impact initiatives.
Looking forward to collaborating with like-minded colleagues driving patient care forward—our patients deserve better.

India’s healthcare system is at a pivotal crossroads. With sovereign and private equity funds like @Temasek , @Khazanah_MY , @Mubadala , @KKR_Co , @generalatlantic , among others making significant investments in healthcare infrastructure, the ecosystem is evolving rapidly.
In this new landscape, nephrologists and other clinicians must adopt a #DigitalFirst mindset, not just in patient care, but in how we design best practices, measure outcomes, and scale operations.

This philosophy has shaped much of my work at @SEHAHealth@SSMCAbuDhabi and in the broader #UAEhealthcare system.
I was also pleased to see the focus on #POCUS (Point-of-Care Ultrasound) in the program's opening session. Just as ultrasound is indispensable to cardiology, obstetrics, emergency medicine, and critical care, it must become a core skill in nephrology to remain relevant and effective. It validates the work we've been doing to embed POCUS across specialties in the UAE.

UAE POCUS Journey - 7 Years in the Making
What started with a borrowed ultrasound machine in @SKMCAbuDhabi dialysis unit has become a movement across UAE healthcare.
The Challenge: Point of Care Ultrasound (#POCUS) wasn't mainstream in medical subspecialities, esp #nephrology, despite being well established in Critical Care and Emergency Medicine for decades.
My early success reducing hospitalizations for dialysis patients related to fluid overload and vascular access was limited - I couldn't institutionalize it working alone.
The Breakthrough: Joining @SSMCAbuDhabi , I launched a training program with the support of Deanne Kashiwagi, Tahir Mehmood, and Hatem Soliman Aboumarie via an @kosmos_echonous research grant.
We became the first Middle East facility with an “AI POCUS Academy”.

The Movement: Something exciting is happening now - Multiple POCUS-trained physicians are now in UAE. This last few weeks alone, I've participated in courses at:
A. SEHA Tawam Hospital -Dr Naren Raj
B. Al Zahra Pvt. Hospital Dubai by Dr. Rachna Sahityani & AbdolGhader Pakniyat,
C Sheikh Khalifa Medical City (Dr. Waqar Gaba, Yousef Alabrach, Emad Khater swalha
The Emirates Society of Internal Medicine now endorses this approach, thanks to Dr.Abdulla Sultan Almarzooqi.
The Vision: POCUS-assisted physical examination should be standard care for undifferentiated shortness of breath and shock - unifying the language between Emergency Medicine 📷 Internal Medicine ( esp Nephrology) 📷 Critical Care.
Unlike my digital transformation work in Nephrology, which required convincing a CEO ,Dr. Gareth Goodier, contributing to the Kidney transplant program growth in the UAE, which involved working with a well-established program at SEHA Sheikh Khalifa Medical City
This required aligning with a community of advocates @NephroP@EchoSoliman@ArgaizR@ASARUC1 , across institutions and many others championing this in their practices.
Now @DubaiHealth and Dr Rasha Bahumaid have started a Master's Program for POCUS, leading certification - a big win for the #UAE Healthcare Ecosystem
Key lesson: Sometimes systemic healthcare change requires patience and grassroots advocacy, not just top-down decisions.
Most satisfying project of my career - seeing gradual adoption has been life-affirming, also that I have trained some passionate people who will lead this program long term
