🧬Can targeting microRNA pathways modify post-MI remodeling & prevent progression to #HF?
��HF-REVERT: microRNA-132 inhibitor CDR132L in patients with reduced LVEF after MI. Presented at #HeartFailure26 by Prof. @JBauersachsMD
📍Phase 2 randomized, double-blind, placebo-controlled trial
📍Primary endpoint (% change in LVESVI at 6 months) improved across groups, but differences were not statistically significant between groups
📍Secondary endpoints including LVEF, NT-proBNP, GLS, & KCCQ also showed no significant differences
📍Exploratory subgroup analyses suggested potential benefit in patients with more advanced adverse remodeling at baseline
📍CDR132L was generally well tolerated with no hepatic, renal, hematologic, or cardiotoxicity signals

🫀 What happens when long-term low-dose #digoxin is suddenly withdrawn?
Blinded withdrawal analysis from the #DECISION trial presented at #HeartFailure26 by Prof. Peter van der Meer:
📍Digoxin withdrawal was associated with a ~7x increase in worsening HF events over just 6 weeks
📍14 worsening HF events occurred after digoxin withdrawal vs only 2 events in placebo withdrawal
📍Withdrawal was accompanied by: ⬆️HR,⬇️ SBP, ⬆️NT-proBNP, ⬇️eGFR
📍⚠️caution when stopping long-term low-dose digoxin in pts with HF & LVEF <=50%

🫀Is there still a role for low-dose #digoxin in modern HF therapy?
Primary results from the #DECISION trial presented by Prof. Dirk van Veldhuisen at #HeartFailure26:
📍Low-dose digoxin did not significantly ⬇️ the primary composite endpoint of CV mortality + total worsening HF events
📍However, there was a non-significant 24% relative ⬇️ in worsening HF events
📍In the as-treated analysis, digoxin was associated with a 34% relative risk ⬇️ in the primary outcome
📍Low-dose digoxin was generally safe & well tolerated, including in women & patients with #AFib
📍High study-drug discontinuation rates ➡️ reduced statistical power
