Wilkins score <8 =success for PBMC. Remember often you are trading MS for MR so If you have >2+ MR Valvuloplasty is not a good option.

Here's the throw in real time. I'm moving a little slower and exaggerating the movements a little bit.
When I first tried this, I immediately appreciated that it was faster and easier than the 'flick' method I was taught 25 years ago (and had used ever since).

Never forget, coronary arteries fill during diastole.
The gradient is defined by
1. Diastolic AoP at the “top of the hill” and
2. LV diastolic pressures at the “bottom of the hill”
A patient with low MAP and very high LVDP / PW may have coronary ischemia.
Unload the LV.
