🚨 KEYNOTE-522 Final Analysis

7.8-year follow-up confirms that adding pembrolizumab to neoadjuvant chemotherapy continues to deliver durable survival benefit in high-risk early-stage TNBC. 🎯

High-risk early-stage TNBC
Neoadjuvant pembrolizumab + chemo → adjuvant pembrolizumab

📊 Key results:
• 7-year EFS: 78.3% vs 69.8%
• HR 0.68

• 7-year OS: 85.1% vs 77.2%
• HR 0.64

💡 Benefit remained consistent across:
• PD-L1 subgroups
• Nodal status
• Disease stage

⚠️ Immune-mediated AEs:
35.0% vs 13.1%

This is now among the longest follow-up datasets for immunotherapy in early TNBC, showing durable cure-oriented benefit extending beyond 7 years. 🔥

7 years later… the median PFS with lorlatinib is STILL not reached in advanced ALK+ NSCLC.

CROWN continues to deliver one of the most durable targeted therapy signals ever seen in solid tumors.

🧬 CROWN Trial
Lorlatinib vs Crizotinib
1L advanced ALK+ NSCLC

📌 Key results:
• Median PFS: NR vs 9.1 mo
• HR 0.19
• 7-year PFS: 55% vs 3%
• No new intracranial progression after 30 months on lorlatinib 🧠

Most striking finding:
Patients progression-free at 24 months had a 79% chance of remaining progression-free at year 7.

44% of patients are STILL on lorlatinib at 7 years.

Despite higher grade 3/4 AEs (77%), discontinuation due to TRAEs remained low:
• 5% lorlatinib
• 6% crizotinib

This is no longer just “disease control.”
For many ALK+ patients, this is beginning to resemble long-term functional remission.

これ、単なる現役イジメじゃないな、考えた官僚は天才だろう。大企業のエリサラや公務員、独法の職員は組合健保の付加給付のおかげで月3万内に収まる。一方で1160万円〜所得者が最も多いであろう医師は、徳洲会や独法除き一部を除き、協会けんぽや医師国保含めて付加給付が無い。つまりオプジーボなど高額新薬の乱用にメタ的な適正圧がかかる。200億を超えるリターンだな。天才すぎる。もう咒術の領域だわ。ワイとてすぐ気づかんかった。

石破内閣の薬価改定には失望を禁じ得ません。中間年改定の廃止ができないどころか、革新的医薬品の薬価引下げのルールを拡大したことで、特許期間中新薬のうち 43％の製品の薬価を引き下げられることになりました。また、これまでの中間年改定において一度も適用されていない新薬創出等加算の累積額控除というルールを適用することで、企業の中には、投資回収計画の見直しを迫られ数百億円もの損失を被る可能性も出てきます。これでは医薬品の安定供給もイノベーション推進の環境整備も実現できません。さらに、厚労省は「創薬支援基金」を作って、製薬企業から強制的に拠出金を集める法案を通常国会に出すとのこと。そして、その基金のトップは厚労省からの天下りの模様。もう今の与党に薬価制度の改善は期待できません。国民民主党が選挙を通じて議席を増やし力をつけるしかありません。
