Important study that could allow differentiating the subset of patients with ME/CFS developed by EBV. This is important as it allows to see how ME/CFS is a heterogeneous group where different pathogens can end up developing the disease, from SARS-CoV-2 to different viruses such as EBV. It could even allow linking the symptoms developed in multiple sclerosis with those of ME/CFS by sharing both IgG antibodies against different EBNA nuclear antigens. As the article points out, this subgroup could benefit from 𝐫𝐢𝐭𝐮𝐱𝐢𝐦𝐚𝐛 for symptom improvement, but they would not be cured because this treatment does not eliminate all the cells with EBV latency.
It is important to note that if these antibodies to EBNA antigens (EBNA1_0005, EBNA3_0577, EBNA4_0566, EBNA6_0025, EBNA6_0488, EBNA6_0752) are being detected, it suggests that these peptides are interacting with the immune system through presentation at HLA-II. This should not be the case, because EBNAs antigens are nuclear antigens and, therefore, should be presented at HLA-I. But these antigens evade presentation in HLA-I and are presented in HLA-II, with the danger of being processed and presented as new antigens by performing a series of modifications in this way, increasing the risk of generating autoimmune diseases. The generation of antibodies is generally associated with the interaction of antigens with B cells and CD4 T cells, and this interaction involves the presentation of antigens at HLA-II. Therefore, we see here again a link in having weak HLA-II alleles against EBV that prevent optimal presentation of these EBV antigens, increasing the risk of poor control of EBV latency cells. Therefore, 𝐭𝐡𝐢𝐬 𝐬𝐮𝐛𝐠𝐫𝐨𝐮𝐩 𝐨𝐟 𝐌𝐄/𝐂𝐅𝐒 𝐩𝐚𝐭𝐢𝐞𝐧𝐭𝐬 𝐰𝐢𝐭𝐡 𝐄𝐁𝐕 𝐩𝐫𝐨𝐛𝐥𝐞𝐦𝐬 𝐚𝐧𝐝 𝐰𝐢𝐭𝐡 𝐞𝐥𝐞𝐯𝐚𝐭𝐢𝐨𝐧 𝐨𝐟 𝐭𝐡𝐞𝐬𝐞 𝐄𝐁𝐍𝐀 𝐚𝐧𝐭𝐢𝐛𝐨𝐝𝐢𝐞𝐬 𝐜𝐨𝐮𝐥𝐝 𝐚𝐥𝐬𝐨 𝐡𝐚𝐯𝐞 𝐭𝐡𝐞𝐬𝐞 𝐰𝐞𝐚𝐤 𝐚𝐧𝐜𝐞𝐬𝐭𝐫𝐚𝐥 𝐚𝐥𝐥𝐞𝐥𝐞𝐬 𝐚𝐠𝐚𝐢𝐧𝐬𝐭 𝐄𝐁𝐕. 𝐂𝐨𝐧𝐜𝐥𝐮𝐬𝐢𝐨𝐧: the next studies that want to differentiate the subgroup of ME/CFS with EBV origin should use these antibodies together with HLA-II alleles. This will allow this group to be identified and removed from the heterogeneous group of ME/CFS that has different causative pathogens. It could even be named differently to identify the causative pathogen of this subgroup, such as "Long EBV".
