This is the 2nd negative triplet study in 1st line metastatic kidney cancer - COSMIC 313 (ipi/nivo +\- cabo) was the other. It means sequencing doublets rather than giving everything upfront will be the standard of care for some time to come. In STELLAR 002 #ASCO25 zanzalintinib nivolumab +/- rela (LAG3) demonstrated promising preliminary dinical actively for the doublet without clear benefit associated with adding LAG3. Triplets and targets beyond PD1 abs LAG3 both appear challanging as with COSMIC313

Corticosteroids for Immune-Related Adverse Events and Checkpoint Inhibitor Efficacy: Analysis of Six Clinical Trials out on @JCO_ASCO
Study investigates the impact of immunosuppressive treatment💊 on survival in patients with various tumor types receiving anti–PD-1 + anti–CTLA-4 therapy🧪, utilizing data from six international phase II/III trials. The analysis included 834 out of 1,959 patients who were treated with corticosteroids for treatment-related adverse events (trAEs), with 10% also receiving second-line immunosuppressants.
✅Results showed that higher peak corticosteroid doses were significantly associated with worse PFS and OS, whereas cumulative corticosteroid dose was not linked to survival outcomes. Due to the limited number of patients receiving second-line immunosuppression, no definitive conclusions could be drawn regarding its impact on survival.
These findings suggest a need to reconsider current irAE management strategies, advocating for the use of lower corticosteroid doses when possible.
