🔴 💊 Treatments in Heart Failure With Mildly Reduced and Preserved EF:
👥13 studies with a total of 29,875
mean LVEF of 56.3%
👉ARNI, MRA, & SGLT2i separately, ⬇️ primary composite outcome 🆚 placebo
👉 combination of ARNI, BB, MRA, and SGLT2i was the most effective
👉Results were similar for CV☠️/HF🏥for ARNI, MRA, and SGLT2i
👉only SGLT2i had a consistent benefit among all LVEF subgroups
👉 SGLT2i +MRA+ARNI
➡️greatest benefit in HFmrEF
➡️robust benefit in LVEF 50%- 59%
➡️marginal benefit in LVEF ≥60%

A hospitalized poet writes a series of poems that seem possible only when one is near death. Grasping his condition through metaphor, he shares with his doctors an evolving understanding of life’s mysteries. Read "Poet as Patient” by Richard Waring: https://t.co/fjtrcCY8PM
An interesting article providing experimental evidence for exercise benefit in restoring autonomic imbalance in aged hypertensive rats. It would also be interesting to see how this could prevent atrial arrhythmias too. #autonomicheart#GPeeps
In March, an Imperial College team published results of AVATAR--a 3-arm trial comparing standard CB AF ablation, AAD, and CB ablation *Without* confirming PV isol
Guess what? There were no diff in symptomatic AF bw the two ablation approaches! Both better than drugs

Why has no one talked about this story? No media coverage.
It's totally provocative b/c it challenges the current practice of confirming PVI . #EPeeps

#EPeeps High frequency stimulation has many limitations. Burst of HRS in atrial refractory period can at least overcome the frequent atrial capture. Here we stimulate the LIGP and induce LIPV PACs that then trigger AF. A valuable tool to consider for GP mapping and #CNA.
@jczerpa@Dr_Markman Yes what was the duration between end of stim to earliest atrial signal? This can often help distinguish true capture or not and also need re-tests for confirmation of ANS triggered ectopy
