Critical review of Nectin-4 in EV-302 vs. Literature needed!🧵
The recent findings from the EV-302 trial #ESMO24, demonstrating high Nectin-4 expression in almost all UC (median H-score >250) are in stark contrast with the literature, which were published independently of the EV development pipeline:
Contrary to studies by Challita-Eid (PMID: 27013195 - affiliated with the company that developed Enfortumab!) @CR_AACR@AACR, Chu et al. (PMID: 34108177) @CCR_AACR, and our own research @CCR_AACR, which demonstrated heterogeneous Nectin-4 expression in UC with absence of membranous Nectin-4 in the range of 20-40%, the EV302 trial's data again suggests a uniform high/ skewed expression of Nectin-4!
Membranous Nectin-4 expression correlates with EV response! https://t.co/bHbVqun45O. This aligns with observations from other ADCs, such as anti-HER2 ADCs, where target expression correlates with therapeutic efficacy. This correlation reflects a well-established biological principle!
This discrepancy raises concerns about the assumption that Nectin-4 is ubiquitously expressed in UC and underscores the need for careful interpretation!
Future treatment strategies for mUC (might) need to account for Nectin-4 status, potentially favoring alternative therapies like chemoimmunotherapy (JB100, CM901) or next-gen ADCs for patients with low or negative expression.
